Parent-Guardian Responsibility Statement
Statement should be read prior to filling out registration form. Parent/guardian initial is required in box on bottom right-hand side of the front of the registration form indicating parent/guardian has read the above statement.
Prime Time VBC Uniform Order Form
Form must be completed and turned in with payment by Monday, March 24th.
Prime Time Parent-Player Agreement Form
Form should be signed by both player and parent and turned in prior to tryouts.
